Permission to Share

Do you give your permission for Temple Israel to share your information with the Charlotte Jewish News and other Jewish Community entities for the purposes of community partnerships (Jewish Federation of Greater Charlotte, Foundation of Shalom Park, Hadassah). We will not share your information for purposes of solicitation. 

Permission to Use Likeness

I hereby irrevocably grant Temple Israel permission to use, in perpetuity, without compensation, my likeness in photographic or other form in any and all of its publications, and in any and all other media, whether now or hereafter existing, controlled, created, arranged, published, disseminated or utilized (collectively, “Published”) by Temple Israel or its licensees. I hereby release Temple Israel from any and all rights, claims, actions, causes of action, damages, and other liability whatsoever; including, without limitation, any right of privacy, right of publicity, or any intellectual property rights (collectively, “Claims”) that I may have or that may otherwise arise out of the use of my likeness.

I give Temple Israel permission to continue charging my account or credit card annually using the method and timing indicated above. Annual dues and fees are subject to change each year. I must notify Temple Israel if I wish to resign my membership and stop payments on my account.

I acknowledge my commitment to supporting the congregation of Temple Israel, and I pledge to fulfill my financial obligations on or before June 30, 2025. __ I agree to the above statement
